How are records reviewed where you work?

Nine questions, about four minutes. This is the research contribution the certificate records. The programme can already say how consistently the five conditions are applied under study conditions; it cannot yet say what record review actually looks like inside working organizations, and that is the gap these answers fill.

Your answers are stored with no name attached. They are written as an anonymous row carrying only the answers, your sector, a coarse organization size and a two-letter country code. No name, no email, no employer and no free text goes into that row.

If you want a certificate, your name and email are written as a separate record that does not contain your answers. The two share nothing that links them. You are not trading candour for a certificate, and nothing you say here can be traced back to you.

Please answer for the organization as it is, not as policy says it should be. A guarded answer is worth less than no answer.
